In this role, your responsibilities will include the evaluation of patients within either inpatient or outpatient settings and the creation of customized treatment plans tailored to each individual's needs. You will administer physical therapy interventions that enhance mobility, strength, balance, and functional abilities. Collaboration is key; you will partner with nurses, physicians, case managers, and fellow rehabilitation specialists to achieve the best possible outcomes for your patients. Additionally, educating patients and their families on therapeutic goals, discharge plans, and at-home exercises will be a crucial part of your role. You will document evaluations, treatments, and patient progress in line with regulatory and facility guidelines. Your participation in care team rounds and interdisciplinary meetings will ensure that our approach to patient care is coordinated and effective. To qualify, you must possess a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program, in addition to holding the appropriate state licensure as a PT. While CPR certification is preferred, specialized certifications in areas like neurological or geriatric therapy will be looked upon favorably. Previous experience is strongly preferred, especially if it involves treating patients with complex medical and mobility impairments. You should also have strong communication abilities and be comfortable navigating digital charting and clinical documentation systems.
