Description Assess patients within inpatient and outpatient environments and craft tailor-made treatment plans Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall functionality Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes Guide patients and their families through therapeutic goals, discharge details, and at-home exercise plans Maintain documentation of evaluations, treatments, and patient progress in alignment with facility procedures and health regulations Engage in multidisciplinary rounds and clinical discussions to ensure cohesive patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
State licensure as a
Physical Therapist CPR certification is preferred Possession of specialized certifications (e.g., neurological, geriatric) is beneficial but not obligatory Skills
Prior experience is highly desirable Ability to manage patients with intricate medical and mobility challenges Strong teamwork and communication skills Comfort with digital documentation and clinical charting systems Benefits
