GLIDE is a nationally recognized center for social justice, dedicated to fighting systemic injustices, creating pathways out of poverty and crisis, and transforming lives. GLIDE’s mission is to create a radically inclusive, just, and loving community mobilized to alleviate suffering and break the cycles of poverty and marginalization. Position Summary:
As the Family Resource Center (FRC), family services are aimed at helping parents manage and recover from challenges, gain knowledge about parenting and child development, and develop the skills and knowledge to support their children’s learning, development and social and emotional competence. The FRC Case Manager will need to communicate, translate, and use their bilingual skills to act as a liaison, advocate, and resource out in the community to provide intensive case management, family advocacy, and crisis intervention for families in the Tenderloin. Collaborating with other GLIDE program staff, schools, community organizations and Protective Service Workers. The FRC Case Manager also offers trauma informed family engagement and perinatal navigation. Together, this essential role provides assessment of the family system and individual children, development of collaborative service plans, referrals to GLIDE programs and other services, and monitoring of progress towards the family’s goals. This position may periodically work non-traditional hours (early evenings, holidays, and weekends). Regular and predictable attendance is essential. Essential Duties & Responsibilities
Using their bilingual language fluency, conduct intake assessment, surveys and enrollment, collect and provide information on community resources and GLIDE services, provide advocacy and coordination to reduce barriers to services, find creative solutions to meet immediate and ongoing needs, and ensure linkage and support to promote health, recovery, and wellness for children, youth, and their families. Determine eligibility and review/assist with applications for entitled benefits and financial resources including but not limited to General Assistance, CalFresh, MediCal, Social Security Disability, emergency rental assistance, and low-cost housing programs. Develop and attend FRC outreach events and build rapport and working relationships in order to be a reliable source of information, reduce barriers to community resources, and improve awareness of GLIDE programs. Collaborates with other providers from other community agencies, Glide program staff, interns, and volunteers to ensure comprehensive and coordinated services. Facilitate and co-facilitate parent groups, specifically for expecting parents and parents with newborns. Lead and facilitate FRC’s weekly Mommy/Daddy & Me group. Deliver aforementioned services in the community, (when contracted) and school meetings. Manage documentation in a timely manner, which includes but not limited to enrollment, various assessment tools, individual/family service plans, follow-up contacts, CFS reports, program evaluations, and service entries in Salesforce and other external program databases. Participate in regular staff meetings, staff training events, supervisory meetings, accept the responsibility for aiding the development of positive team relationships as requested, and adhere to agency policies, procedures, and professional code of ethics. Qualifications
Must have fluency with reading and writing proficiency in English and Spanish in order to address the needs of the targeted population. Be able to grammatically correct correspondence, reports, comprehensive assessments, service plans, and progress notes in both English and Spanish. Successfully convey and obtain accurate and complete information in a respectful and caring manner, including nonverbal communication or communication with those with language barriers or disabilities. General knowledge and experience with comorbid and chronic mental health disorders from depression, anxiety, trauma, addiction, grief/loss, and psychosis. Commitment to strength-based values where all people have goals, talents, and determination within an environment of community, resources, and opportunities to support the process of recovery. Has good attention to detail, organizational skills, and time management. Knowledge of roles and responsibilities as well as your own strengths and weaknesses that support working independently and collaboratively. Demonstrates reliability, dedication, and integrity in learning, taking action, and meeting goals. Experience or ability to establish cooperative relationships with various professionals as well as formal and informal community members and resources. Ability to adapt to the cultural, familial, linguistic, political, and community context in order to better understand how cultural factors figure into risk and resilience. Adheres to laws, ethics, and values that ensures every individual receives and is provided effective and efficient care to attain or maintain wellness and recovery. Bachelor’s degree in psychology, social work, or other health and human services with 2+ years of sufficient relevant work and/or life experience. Experience working with children, youth and families in a community based social service agency. Possess an understanding of the client’s community and a basic understanding of bio-psycho-social-spiritual and economic issues that might impact families/clients. Experience developing and leading workshops and activities for families/adults; and General knowledge of current local, state and national resources, services, and benefits programs, including services and programs for families with minor children. Must have or complete CPR and First Aid Training. Must pass tuberculosis screening. Must show proof of pertussis (Tdap), influenza and MMR immunizations. Work Environment
GLIDE’s buildings are located in the Tenderloin neighborhood. GLIDE maintains an "open door" policy to the community and its clients, who frequently enter and leave the building. Physical Requirements
Ability to work on a computer and see details of objects at close range. Ability to hear within normal range, and communicate effectively (in person, telephone or Zoom). Finger dexterity and the ability to use all standard office equipment. Sit or stand comfortably; and the ability to navigate throughout office spaces (via elevator or stairs). Salary: $31 - $31.50 an hour This is a fulltime (40 hour/week) Non-Exempt position. This role is covered by the collective bargaining agreement.
