The role you’ll contribute
Responsible for the planning, coordination and evaluation of the services provided to patients within the cardiac and stroke operations throughout the continuum of care and across multidisciplinary lines of services. Ensure THH remains in compliance with all regulatory requirements to maintain Stroke & Chest Pain Disease Specific Certification. Ensure quality and safe care is delivered to chest pain & stroke patients by presenting and implementing current guidelines and evidence-based practice for cardiac patients. Coordinate and supervise all activities related to Huguley’s cardiovascular databases. Maintain an efficient and accurate method of collecting, processing, and distributing CV-related data in a consistent manner, while maintaining data quality. Coordinate and participate in Quality Improvement of CV-related processes based on data outcomes. Ensure organizational and community integration of cardiac/stroke services, education, research, and outreach to improve care and maximize outcomes. Build and maintain strategic relationships and customer loyalty among physicians, employees, and the community. Demonstrate critical thinking and independent decision making using policies, procedures, standards of care, and data analysis. Collaborate and communicate with physicians to ensure quality care. Plan for appropriate patient populations and manage the plan of care with the interdisciplinary team through ongoing collaboration and use of care plans. Develop, utilize, and model effective time management skills. Communicate with all team members regarding therapeutic interventions including patient, family, and significant others. Set patient care priorities and manage total patient care, including invasive and non-invasive monitoring. Develop and coordinate the education plan appropriate to the patient, significant other, and family learning style. Initiate and communicate changes in patient care delivery based on continual evaluation of progress according to the outcome-based care plan. Collect data to analyze outcome-based care management variance reports to evaluate patient progress. Coordinate discharge and outpatient plan of care with patients and family. Other duties as assigned or requested. Qualifications
BSN Required; MSN preferred Cardiovascular RN with a minimum of 3 years Cath Lab experience required Abstraction Experience RN licensed in the state of Texas or a qualifying compact state
