Vitra's Adult Day Health (ADH) center in Brockton, MA is a daily care facility designed to meet the physical, functional, and social needs of seniors or individuals with disabilities, including those with complex medical or rehabilitative needs.

Essential Functions
- Assist with intake and annual assessments by organizing forms, tracking deadlines, and maintaining client charts.
- Prepare documentation of care plans, interdisciplinary team meetings, and external communications.
- Schedule and track client appointments, facility visits, caregiver education, assess psychosocial and environmental changes, and activity events.
- Assist with creation and distribution of monthly activity calendars, newsletters, and program flyers.
- Maintain accurate records of activity participation, caregiver interactions, and psychosocial updates.
- Help coordinate logistics for field trips, special events, and transportation arrangements.
- Support supply inventory tracking and assist in ordering materials for program activities.
- Answer phones, greet visitors, and route communications professionally and efficiently.
- Maintain confidentiality of sensitive information and adhere to all HIPAA and company privacy policies.
- Perform other administrative duties as assigned by management.
Education, Experience and Licensure Requirements
- Bachelor's degree in human services, public health or another related field required
- Willingness to travel within assigned region required
- Valid driver's license and reliable transportation required
- Minimum of two years recent experience in the direct care of elders or adults with disabilities required
- Bilingual in English and Cape Verdean Creole strongly preferred
- Comprehensive knowledge of case management principles, healthcare management and reimbursement
- Case management, behavioral health, social services and/or community health worker experience preferred
- Proficiency in Microsoft Office (Word, Excel, Outlook) and comfort using electronic documentation systems.
Physical and Environmental Demands
- Requires frequent sitting, walking, standing, and use of hands for computer and office tasks.
- Requires lifting and carrying supplies or equipment weighing up to 35 pounds.
- Requires ability to push/pull carts or assist with event setups.
- May require navigating stairs and transporting light materials within the facility.
- Works in a clean, well-lit environment with fluctuating temperatures and proximity to others.
