💼 What You’ll Do:

As the Director of Social Services, you will:

• Provide emotional support, counseling, and advocacy to residents and their families
• Lead care planning and discharge coordination with the interdisciplinary team
• Ensure compliance with all state and federal regulations
• Serve as a liaison between residents, families, and healthcare providers
• Promote resident rights and ensure a compassionate, respectful environment
• Address and resolve concerns with professionalism and empathy
• Support quality of life and resident satisfaction initiatives

✅ What We’re Looking For:

• Licensed Social Worker (LSW/CSW) – active and in good standing
• Bachelor’s degree required, Master’s degree strongly preferred
• Experience in long-term care, subacute, or a healthcare-related setting
• Knowledge of nursing home regulations and compliance standards
• Strong communication, conflict resolution, and interpersonal skills
• COVID-19 vaccination (primary series) required
