Ambulatory Clinical Pharmacy Specialist-Gastroenterology
The Ambulatory Clinical Pharmacy Specialist is responsible for ensuring optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Provides drug information to patients and families, physicians, nurses, and other healthcare professionals. Integrally involved in education of pharmacists, residents and students. Serves as a resource and drug expert in the gastrointestinal specialty. Essential Responsibilities / Duties
Responsible for pharmaceutical care within the gastroenterology ambulatory care practice setting. Be a resource to oversee quality improvement projects, formulary decisions and other workflow designs. Develop a practice within the various ambulatory practice sites and liaison as appropriate with other partners. Evaluate, monitor and modify drug therapy, provide recommendations to optimize patient outcomes, provide patients with new medication education, minimize safety issues and assure cost effectiveness as well as provide drug information for medical personnel. Collaborate with various stakeholders to ensure accreditation and/or statutory requirements are being met. Evaluate, monitor and modify drug therapy, and, if applicable, prescribe or discontinue treatments when necessary and when outlined under the collaborative practice agreement. Provide patients with new medication education. Coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting. Participate in medication formulary review. Assist with collection and presentation of patient outcomes data. Provide clinical support in gastroenterology and other specialty disease priorities as needed. Education and Job Requirements
Work requires at a minimum a Bachelor's Degree in Pharmacy from an accredited college of pharmacy and at least five years of pertinent work experience; or a Bachelor's Degree in Pharmacy from an accredited college of pharmacy and post-graduate training, with at least three years of pertinent work experience; or a Doctorate Degree in Pharmacy from an accredited college of pharmacy with post-graduate training or at least two years of pertinent work experience. Work requires licensure to practice pharmacy in the state of Massachusetts by the hire date. Must be eligible for credentialing as a licensed independent provider practicing under a collaborative drug therapy management agreement.
