What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan; work with PTAs to follow the care plan. Help patients with mobility, movement, and regain independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality of care. Wage range for Physical Therapist, Level 1: 120,000.00-135,000.00 plus 10K annual bonus, paid quarterly. Starting pay commensurate with relevant experience. 21st Century offers generous bonus plans, health benefits, and 401k (up to 4% match). As a practice, PT Case Managers are often hired as PTIs; progression to PTII with increased salary and bonus is possible.
