Responsibilities
- Perform and document highly complex social services work, serving as a liaison between patients, legally authorized representatives, and others to assure ongoing care, treatment, and support through person-centered practices.
- Provide individuals, families, and groups with psychosocial support to cope with chronic, acute, or terminal illnesses; gather information to assess an individual’s support systems and service needs; participate in interdisciplinary team meetings as needed.
- Assess an individual’s rights and capacity to make decisions; design and provide individual and group trainings to promote rights and patient transition, including curriculum and materials.
- Assist with coordination of furloughs, transfers, transitions, and discharges as needed.
- Interview and converse with individuals, legally authorized representatives, and involved persons to maintain the individual’s social history and identify psychosocial, economic, and physical needs, with updates as appropriate.
- Coordinate communication and information exchange with involved parties and providers to assure ongoing care and support, including required notifications and consent where needed.
- Counsel and assist individuals and representatives regarding community placement issues, living options, end-of-life planning, and rights in accordance with state and federal laws and policies.
- Provide information, counseling, and referrals for other services; deliver case management or interventions to promote health and address barriers to access to care.
- Design and conduct individual and group trainings to promote rights, decision-making, person-centered thinking, and self-advocacy skills; develop curriculum and training materials for patients and stakeholders.
Qualifications and KSAs
- Requires Bachelor’s degree (or foreign equivalent) in Psychology, Social Work, or a related field.
- Three (3) years of experience in the job offered or related occupation; one (1) year of experience working directly with persons with mental, social, or substance use disorders.
Registrations, Licensure, and Certifications
- Maintain licensure according to the Texas Board of Social Work Examiners; Licensed Baccalaureate Social Worker (LBSW).
