Part-time Admission Nurse Schedule
Every Friday, Saturday or Sunday OR Every Saturday, Sunday or Monday. Saturday/Sunday schedule may also be considered. What you’ll do
Utilize clinical skills to complete the initial assessment of patients to determine home care needs, including: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow-ups and evaluation visits in the designated territory Make home visits to patients in the designated territory Reconcile medications, perform wound care, treatments, and procedures as assigned Monitor patients’ conditions and report changes to the Clinical team Collaborate with physicians and agency staff to deliver optimal care Initiate and administer IV medications (antibiotics and diuretics) infusions safely and in accordance with established protocols Accurately document observations, interventions, and evaluations of patient care To be a success
Current and unrestricted CA RN license Current BLS/CPR Certification Valid CA Driver's License Experience conducting SOC - OASIS is necessary High level of clinical knowledge in home health care is highly preferred Experience with high-acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Previous experience with IV medication infusions is highly preferred Proficiency initiating intravenous (IV) therapy is a plus Self-directed, empathetic, critical thinker with outstanding interpersonal skills Leadership and organizational skills Equal Opportunity : 21st Century is an equal opportunity employer, committed to fostering a diverse and inclusive workplace.
