Part time Registered Nurse Case Manager responsibilities include conducting Start of Care (SOC) Admissions - OASIS, follow ups and evaluation visits in the designated territory, home visits, medication reconciliation, wound care, treatments, procedures, monitoring client conditions and reporting changes to the clinical team, collaboration with physicians and agency staff, and accurate documentation of observations, interventions, and evaluations. Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols. Additional territory details: Anticipated territory Santa Clara County; Schedule: Saturday/Sunday. Qualifications
Current and unrestricted CA RN license Current BLS/CPR Certification Valid CA Driver's License Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired High level of clinical knowledge in home health care; experience with high acuity patients is a plus Experience working with an interdisciplinary team; Case Management fundamentals are highly desired Previous experience with IV medication infusions is highly preferred; proficiency initiating IV therapy is a plus Self-directed, empathetic, critical thinker with outstanding interpersonal skills and leadership abilities Organized with the ability to manage multiple responsibilities Why join us
