Role Integrated Team Counselor - Mobile
(ITC) — Adult Community Clinical Services (ACCS) Integrated Teams provide clinical interventions and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management. Working as part of the ACCS team under the guidance of the Team Leader, staff will provide services and interventions identified in the Treatment Plan, including education, supervision, and direct supports to enable individuals served to maintain physical and emotional health, live safely in the community, and develop skills to meet their goals. Interactions honor the individuals served as unique adults, with a goal of mutual learning and growth. Services align with Riverside and Department of Mental Health standards and requirements.
In This Vital Role, The ITC Will
Provide input to person-centered assessment and service planning, advocacy, coordination, and linkage with other services and the community. Together with other team members, provide rehabilitation interventions, including skills training in community environments.
Provide education, training, and, when needed, physical intervention to help persons served learn life skills including budgeting, banking, transportation, housekeeping, personal hygiene, self-medication, nutrition, and health.
Model and teach skills for developing positive relationships and negotiating conflict.
Assist persons served with accessing necessary and appropriate health care.
Maintain regular clinical records in keeping with DMH, agency, and division policies and procedures, and within required timelines.
Consistently perform all medication administration and documentation procedures in accordance with Riverside and State policies and regulations related to the Medication Administration Program (MAP).
Schedule Full Time, 40 hours
Tuesday through Saturday
Compensation Pay Rate:
$21.37/hour

Required Experience
Bachelor’s Degree preferred or High School diploma required
