Job Summary
The Licensed Clinical Social Worker provides comprehensive clinical social services to individuals and caregivers in their place of residence. The Social Worker 2 provides clinical social services to individuals and families in their place of residence.
Responsibilities include:
- Conducting biopsychosocial assessments
- Providing crisis interventions, supportive counseling, and grief counseling
- Developing and implementing treatment plans
- Collaborating with interdisciplinary teams
- Participating in quality improvement initiatives
Minimum Qualifications
For Licensed Clinical Social Worker:
- California Licensed Clinical Social Worker (LCSW) license
- Master's Degree in Social Work from an accredited school
- Experience in an acute medical hospital, mental health setting, or healthcare clinic
- Basic computer skills and ability to utilize medical terminology
For Social Worker 2:
- Valid CA driver's license
- Master's Degree in Social Work from an accredited school
- Home Health and/or hospice experience
- Knowledge of human behavior, psychotherapy, and communication
Preferred Qualifications
Specialty social work certification, prior home health or hospice experience, and proficiency with electronic medical records are preferred.
Work Environment
Work environment may include clients' homes, hospitals, care facilities, and clinics. Must be able to travel independently to these locations and participate in the DMV pull notice system.
