Join UC Davis Health as a Licensed Clinical Social Worker or Social Worker (Weekend Social Worker). The selected candidate may be hired at either Licensed Clinical Social Worker or Social Worker 2 level, based on qualifications.

Job Summary

Licensed Clinical Social Worker: Provides comprehensive clinical social services to individuals and caregivers in their place of residence. Functions as an integral member of the interdisciplinary team, promoting, maintaining, or restoring health, or minimizing the effects of illness and disability.

Social Worker 2: Provides clinical social services to individuals and families in their place of residence, promoting, maintaining, or restoring health, or minimizing the effects of illness and disability. Works collaboratively with physicians, nurses, and other allied health professionals, supporting patients with conditions that span the entire health continuum, including end-of-life care.

Responsibilities

  • Provides a full range of evidence-based social work services to patients in Home Health, Hospice, and Home-Based Palliative Care.
  • Conducts biopsychosocial assessments, crisis interventions, supportive counseling, and grief counseling.
  • Address social, financial, and psychological problems related to patients' health conditions, facilitating patient movement through the health continuum.

Requirements

  • Licensed Clinical Social Worker: Master's degree in social work, California Licensed Clinical Social Worker (LCSW) license, and experience in an acute medical hospital, mental health setting, or healthcare clinic.
  • Social Worker 2: Master's degree in social work, valid California driver's license, and experience in home health and/or hospice.

Preferred Qualifications

  • Specialty social work certification, proficiency with computer programs, and experience with electronic medical records (EMR).
  • Prior experience in home health, palliative care, and/or hospice social work.

Work Environment

Work environment may include clients' homes, hospitals, care facilities, and clinics. Must be able to travel independently, using a personal vehicle, and participate in the DMV pull notice system.
