Overview

Patient Engagement Specialist I - LGBTQIA+ Outreach Focus. Under general supervision, provides patient engagement services to recruit patients into the program's system of care. The role participates in planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements. Focus on connecting the medically underserved to community based healthcare services. Responsibilities

Develops, identifies, and provides referrals within and outside of FHCSD, and effectively monitors and tracks such referrals. Effectively communicates with and recruits new patients into the clinic’s or program’s system of care, interacting with community members and patients from diverse backgrounds. Leverages community resources to raise awareness of FHCSD; maintains relationships with community partners and connects patients with FHCSD. Participates in and supports planning, development, execution, and evaluation of patient engagement plans; investigates leads for recruitment and engagement opportunities. Performs other duties as assigned. Written work (plans, flyers, reports) is accurate and timely; uses computer resources to meet organizational needs. Education/Certifications/Licenses/Registrations

At least 1 year of experience in sales, patient engagement, community education, community health, or outreach programs; program-specific experience may be required. Associate's Degree in psychology, sociology, social work, communication, public health or related field strongly preferred; relevant experience may substitute. High School diploma or GED required or equivalent combination of education and experience meeting job duties. Travel between sites is required; must have a car, a valid California driver's license, proof of insurance, and acceptable driving record. Experience / Specialized Skills (including Language)

Ability to maintain good interpersonal interactions within a multi-disciplinary team in a diverse population. Ability to multi-task, adapt to changing priorities in a fast-paced environment. Basic computer literacy (database and Microsoft Office). Bilingual English/Spanish preferred. Strong communication skills: active listening, paraphrasing. Ability to investigate leads for recruitment and engagement opportunities; intermediate level. Knowledge of San Diego County community resources and health/social services; ability to establish partnerships. Salary range: $23.00 - $27.46 hourly, exclusive of benefits.
