Provides age-specific, quality, patient-centered care to all patients using the nursing process and standards of nursing practice with sensitivity and respect for diversity. Expedites appropriate care and treatment with critical reasoning and clinical judgment, especially for complex or potentially life-threatening conditions. Assesses, triages, and provides nursing consultation; performs follow-up and care coordination during face-to-face or telecommunication encounters. Applies nursing interventions: identifying and clarifying patient needs, conducting health education, promoting advocacy, coordinating services, assisting patients to navigate the health care system, and evaluating outcomes. Maintains patient safety and high-quality nursing care during each encounter. Interacts with patients through in-person and telecommunication modalities, often establishing long-term relationships. Acts as partner, advocate, and advisor to support patients/families in managing health care needs, respecting culture, values, goals, and preferences. Facilitates continuity of care using the nursing process, interprofessional collaboration, and access to appropriate services and community resources. Collaborates with inpatient and clinic staff to develop educational materials and assess learning needs, developing curriculum/content accordingly. Demonstrates leadership, promotes team building, and participates in staff development and competency evaluation. Maximizes productivity, completes reports and projects on schedule, and manages priorities effectively. Engages in lifelong learning to update clinical, organizational, and professional knowledge and skills; uses performance improvement methods to design data collection systems and identify opportunities to improve care. Other duties as assigned, including On-Call responsibilities. Education Qualifications

Bachelor’s degree or equivalent from an accredited college/university (Required). Experience Qualifications

Two (2) years of progressively responsible and directly related RN work experience. Required Knowledge, Skills And Abilities

Knowledge of the specialty area of practice and of current nursing theories, standards, and practices. Proficiency with computer systems and software used in the assigned work area. Understanding of laws, regulations, standards, and guidelines governing nursing care, including ethics and patient privacy rights. Knowledge of medical terminology, health promotion, risk reduction, disease prevention, medications and dosing concepts, and adverse reactions. Strong teaching, problem-solving, critical thinking, and communication skills; ability to translate medical terminology for patients and interact effectively in challenging situations. Ability to plan, coordinate, and guide patient care with the interdisciplinary team and to supervise non-licensed personnel in the area of responsibility. Ability to evaluate and modify patient care plans to meet individual needs. Licenses and Certifications

Base Pay Scale: Generally starting at $74.73 - $99.04 per hour. Final salary determined based on internal equity, experience, education, and other factors. This pay scale is not a promise of a particular wage.
