Access Community Health Network is hiring for a Physician Assistant/Family Nurse Practitioner. This is a full-time position with base pay range listed below and a comprehensive benefits package. Base pay range $95,000.00/yr - $115,000.00/yr

Position summary Physician Assistant / Family Nurse Practitioner - Position Summary

Provide the full scope of services within the provider’s field of training, including diagnosis, treatment, care coordination, preventive care, and electronic health record (EHR) maintenance, while adhering to Access Community Health Network quality management, risk management, infection control, customer service and safety guidelines.

Clinical and operational duties General Clinical

Obtain health history and perform physical examinations.

Diagnose, treat, counsel, and coordinate care for patients.

Review and update problem lists and longitudinal plan of care at every visit.

Meet documentation standards and complete all progress notes within 24 hours of visits.

Fulfill on-call responsibilities and respond to calls promptly per ACCESS schedule.

Prescribe medications in accordance with Illinois statutes and professional guidelines.

Ensure activities align with ACCESS quality standards and clinical guidelines.

Meet Meaningful Use standards with certified EHR technology for improved care quality and coordination; maintain patient privacy and security.

Manage EHR In-Basket activities and respond to messages within 72 hours.

Ensure clinical activities comply with reimbursement guidelines.

Patient Support & Education

Provide culturally competent care and involve families as part of the care team where appropriate.

Meet patients’ and families’ needs and follow clinic policies.

Encourage questions and patient self-management; conduct education for pre/post instructions and treatment plans.

Utilize printed materials and tailor education to patient understanding; coordinate with care team for case management and appropriate transfers when needed.

PCMH Requirements

Coordinate with health center staff to meet patient needs; participate in huddles and productivity standards.

Ensure timely and appropriate patient visits with involvement of medical assistants and front desk staff as needed.

Ensure care teams perform tasks per policy and procedure.

Provider Development

Participate in staff orientation and training; assist with certification requirements for patient education and medication administration.

Participate in proficiency testing to maintain care standards; may act as a preceptor for learners.

Adhere to HRSA and FTCA regulations.

Other duties as assigned.

Requirements / Preferences

Current PA license in the State of Illinois; CME requirements ongoing.

Board Certification required.

Obtain and maintain a collaborative agreement at start of employment; complete Practice Reviews as required.

DEA license at start of employment.

Two years of outpatient primary care experience preferred.

Knowledge of relevant safety and infection control regulations; EPIC EHR knowledge preferred.

The pay range provided represents the minimum to maximum for this position. Actual compensation varies based on experience, education, market conditions, and available grant funding.
