Compensation Base pay range:
$95,000.00/yr - $115,000.00/yr
Benefits and compensation details include:
Compensation Package: Guaranteed Annual Base Salary; Productivity and Quality Bonuses; Relocation Assistance
Loan repayment potential through National Health Service Corps (NHSC)
NHSC LRP: Up to $75,000 Full Time for 2 years
NHSC SUD-W LRP: Up to $75,000 Full Time for 3 years
NHSC LRP or SUD-W LRP: $5,000 award enhancement for Spanish-language proficiency
HPSA Primary Care score 19 for ACCESS Health Centers
ACCESS Westside, Madison and Sinai Health Centers have a HPSA Primary Care score of 21

Clinical and Operational Duties include a broad range of responsibilities within General Clinical, Patient Support & Education, PCMH Requirements, and Provider Development domains as described below.
Clinical and Operational Duties
General Clinical
Obtains health history and performs physical examinations
Diagnoses, treats, counsels, and coordinates care for patients
Reviews and updates problem list and longitudinal plan of care at every visit
Meets documentation standards and completes progress notes within 24 hours of visit
On-call responsibilities with responses within 20 minutes as scheduled
Prescribe medication in accordance with Illinois statute and guidelines
Adhere to quality standards and clinical guidelines established by ACCESS
Meet Meaningful Use standards using certified EHR technology to improve care quality and coordination
Manage EHR In-Basket and respond to messages within 72 hours
Ensure clinical activities align with reimbursement guidelines
Patient Support & Education
Provide culturally competent care
Recognize families as part of the care team and meet patient/family needs
Encourage questions and patient self-management
Conduct patient education and document instructions
Coordinate with care team and case management to deliver integrated services
Transfer patients with greater acuity appropriately
PCMH Requirements
Coordinate with health center staff to meet patient needs
Participate in huddles, maintain productivity standards, and ensure timely visits
Engage front-line staff in care processes and ensure policy/conduct aligns with procedures
Provider Development
Participate in orientation and training of staff as needed
Ensure care team meets internal certification requirements and participates in proficiency testing
May serve as a preceptor for residents and students
Adhere to HRSA and FTCA regulations
Other duties as assigned
Requirements/Preferences
Current Professional Licenses to practice as a PA in Illinois; maintain CME requirements
Board Certification required
Collaborative agreement upon starting employment; complete Practice Reviews as required
DEA license and Controlled Substances license upon beginning employment
Two years of outpatient primary care experience preferred
Knowledge of quality, safety, and infection control standards
Electronic Health Records competency; EPIC knowledge preferred
Competencies/Behaviors
Cultural Competency and ability to serve diverse patient populations
Interpersonal and administrative/organizational skills
Working Conditions/Equipment
Clinical/Health Center environment with concentration and communication in interruptions
Position may require standing, sitting, and walking for extended periods
Computer/laptop and standard office equipment
ACCESS is a Network of Federally Qualified Health Centers. Depending on the position, candidates may be required to provide vaccination documentation or apply for exemptions as part of the application process.
The pay range shown represents the minimum to maximum for this position. Actual compensation varies based on experience, education, market conditions, and available grant funding.
