County Durham & Darlington NHS Foundation Trust is seeking a part-time Consultant in Palliative Medicine (7 sessions initially) to join our existing team. The post will be based in Derwentside and University Hospital of North Durham with elements of acute, community and home-based care. The closing date is 11 October 2025. The Specialist Palliative Care team includes a Macmillan Palliative Care Lead Nurse, Palliative Care Consultants, Macmillan Clinical Nurse Specialists, Macmillan Discharge Facilitators, and Hospice partners. The successful candidate will work with key partners across County Durham and Darlington to deliver high-quality specialist palliative care. The following job summary defines the scope of palliative care consultant work in CDDFT. The exact duties will depend on the job plan agreed with the successful applicant and may be setting-specific. Provide clinical leadership and expert knowledge in palliative medicine for CDDFT. Collaborate with other palliative care consultants and colleagues in primary and secondary care to ensure high-quality palliative care in line with national and Northern England Clinical Strategic Network standards. Work as part of the Specialist Palliative Care Consultant Team to provide cover for all settings in County Durham and Darlington, including: Clinical leadership for community-based specialist palliative care teams and senior nurses in care homes. Consultant support and advice for primary health care teams and prisons as needed. Consultant support and supervision for medical staff and patients within inpatient palliative care. About us

Duties across all settings Function as an essential member of the specialist palliative care teams throughout the county. Provide expert advice on complex ethical and clinical decision-making. Collaborate with other palliative care consultants to provide expert advice, support and clinical expertise, Monday to Friday 9–5 pm, and contribute to the trust-wide palliative care telephone advice rota. Contribute to a 1 in 7 commitment to the Out of Hours Advice Service for professionals. Undertake administrative duties related to patient care in line with Trust policy. Hospital setting Provide expert advice and support to health care professionals caring for palliative and end-of-life care needs. Provide clinical leadership and support for the hospital palliative care team. Community setting Provide expert advice and support to professionals caring for palliative and end-of-life care needs in community settings (specialist palliative care teams, primary care teams, community nurses, care home staff). Lead the specialist palliative care multidisciplinary team (MDT) and clinical review meetings as required. Undertake domiciliary visits to patients in their usual place of residence (including community hospitals, care homes, and prisons) as required. Work with community hospital clinicians to identify, assess and review palliative care needs. Support and attend Primary Care Team palliative care register meetings as appropriate. Hospice Provide expert advice and support to the hospice multidisciplinary team through weekly ward rounds and inpatient unit MDTs. Provide leadership to the multi-professional specialist palliative care team, including hospice teams and primary care colleagues. Provide supervision of other doctors within the service (including appraisal). Lead governance, service development and change initiatives across local networks as appropriate. Contribute to clinical governance activities, audits, and development of standards; participate in local and national audits and quality improvement initiatives; contribute to risk management and patient safety; contribute to policy implementation and appraisal/revalidation processes for palliative medicine colleagues. Maintain ongoing professional development in line with Royal College requirements; contribute to undergraduate and postgraduate education in palliative care and ethics; participate in specialist palliative care service education sessions. Identify and develop research proposals; foster links with academic centres to support local research and implement appropriate projects. Qualifications and Experience
Special Skills & Knowledge
Full GMC Registration; MRCP or MRCGP or equivalent. Experience on the GMC specialist register or within 9 months of registration for attaining CCT in palliative medicine at time of interview. Extensive experience in palliative medicine across hospice, hospital, and community settings; ability to work autonomously and lead multidisciplinary teams; excellent communication and teaching skills; strong leadership and management abilities. IT literate; experience in clinical audit and teaching; evidence of service development and research activity. Special Requirements
Capable of independent travel across the county. Statutory Registration
