Responsible for facilitating a variety of substance use disorder coaching and support services, as well addiction treatment and education services, addiction treatment case management services with Permanent Supportive Housing clients as a part of a Specialized Intensive Community Treatment (SICT) Team. Successful candidate must understand recovery and psychosocial rehabilitation, and harm reduction, principles, and adhere to Rule 2060.Will work as a part of an integrated primary and community health outreach team to implement CCHC’s intensive community engagement services. Responsibilities Include but are not limited to: Use an integrated approach as a part of a care team Use evidence-based tools and other standards of care (e.g., Screening, Brief Intervention, and Referral to Treatment (SBIRT), CAGE, DAST, AUDIT etc. Provide case management services to patients with mental health disorders to support addiction treatment, including coordination with specialty providers, recovery homes, inpatient treatment and community based providers, that will provide care via referral agreement for severe/complex cases, if applicable. Provide integrated case management services to patients with co-occurring mental health and substance use disorders to support treatment, including coordination with specialty providers that will provide care via referral agreement for severe/complex cases, if applicable. Expand evidence-based mental health and substance abuse prevention and education programs for patients, families, communities, and personnel to increase awareness of, patient access to, and patient retention in mental health and substance use disorder treatment programs. As part of a comprehensive approach to housing stability and community re-integration of chronically homeless populations, reducing risk for people struggling with addiction, increase awareness of the appropriate use of naloxone to reverse opioid overdose through patient, family, community, and personnel training and other prevention or harm reduction interventions. Empower clients with mental health and substance use disorders to make informed decisions about their care, including pain management alternatives, treatment options, and recovery through peer counselling, patient education, or other evidence-based strategies. Document and maintain current and accurate records and program forms for reporting, billing and patient documentation purposes. Close patient charts timely as outlined by CCHC policies, and adhere to counseling regulations and standards Rule 2060. Enhance education for patients, families, communities, and personnel to support patient engagement and self-management that includes medical conditions that often co-occur with mental health and substance use disorders (e.g., diabetes mellitus, heart failure, hepatitis, HIV/AIDS, hypertension, obesity). Collaborate with existing community resources to address environmental factors that impact the onset or recurrence of substance use disorders, with a focus on opioid use disorders Provide support and motivational interviewing to clients addressing behavioral and/or psychosocial issues in keeping with best practice standards. Provide case management, and outreach services in the home, service providers, inpatient treatment support and office setting Completes screenings, intakes, case management and scheduling and referrals Ensure that all recording and reporting of client contact is accurate, up to date and of good quality, working within the PCMH/JCAHO accrediting standards. Provides alcohol/DUI, mental health, primary care and related service referrals and coordination as needed. Facilitate didactic counseling and educational group sessions Coordinate services with case management and medical team in accordance with established procedures using prescribed forms. Maintain and submit weekly/monthly statistical reports related to program services performed (service referrals received, [internal and external] depression screens, domestic violence screens, individual counseling sessions conducted, etc.) Document all client encounters and contacts made on behalf of clients utilizing prescribed forms; develop, complete and submit weekly progress reports. Participate in multi-disciplinary staffings, program and clinical meetings, and participate in peer review and staffings, quality and organizational trainings as outlined by direct supervisor. Helping participants in setting individualized goals for themselves and in assessing their own skills and resources related to goal attainment. Goals are set by exploring strengths and needs in the individual’s living, learning, social, and working environments. Implementation of services in the plan may take place individually or in groups. Perform other duties as assigned. Minimal Qualifications/ Experience/ Skills:

High School Diploma, Associates or Bachelors in social services preferred, addictions studies or related field preferred. Must have expertise in working with homeless populations and people with substance use disorders. Experience in working with patients with substance use or dependence diagnosis and with chronically homeless populations required. Employee Benefits offered to Fulltime/Exempt Staff
