Overview
Join Avow, Inc. as a Certified Nursing Assistant (CNA) in Fort Myers, FL. This role supports hospice patients and families with personal care and related services as Avow expands into Lee County.
Responsibilities
- Provide direct, personalized care based on the patient’s Aide Care Plan determined by a Registered Nurse; report promptly to RN for reassessment if care plan is unclear or patient condition changes.
- Assist the patient with personal hygiene and independence in ADLs according to abilities and condition.
- Offer emotional support to patient and family; communicate with patient/family with respect and dignity; document interactions timely and accurately on every visit/phone call.
- Coordinate with Registered Nurse or clinical staff to provide and replenish medical supplies and equipment.
- Report changes in patient’s mental, emotional, or physical condition to the interdisciplinary team as directed.
- Complete documentation accurately and in a timely manner; document visit actions and interventions with reasoning.
- Participate in team functions, IDG meetings, organizational meetings, and continuing education when possible.
- Perform other duties as needed; complete required continuing education and annual competency tasks; maintain quality of care, infection control, compliance, and customer service initiatives.
- Adhere to scheduling communications; notify supervisor of variations; perform other duties as assigned.
Qualifications
- Florida Certified Nursing Assistant License (CNA) required; in good standing on the Florida State Nurse Aide Registry.
- High school diploma or GED preferred; 1-3 months related experience or training or equivalent combination; successful completion of a nurse aide training program meeting state and federal requirements.
- Valid Florida Driver's License; CHPNA recommended after 500 hours or 1 year of employment (before two years) as criteria met.
- Valid automobile insurance; valid Basic Life Support (BLS).
- Computer skills including database software and general email/tech literacy as outlined by the organization.
- Language: English (reading, writing, speaking); bilingual in Spanish a plus.
- Physical demands: ability to lift/move up to 50 pounds; ability to sit, stand, walk, reach; ability to perform required activities with reasonable accommodations.
