This is a full-time on-site role for a Licensed Clinical Social Worker located in Saugus, Danvers or Amesbury. The Licensed Clinical Social Worker will be responsible for providing direct social work services to residents and their families, including medical social work, case management, mental health, and behavioral health support. Duties include assessing and addressing the social and emotional needs of residents, coordinating care plans, providing counseling, and offering resources and support to enhance residents' quality of life.

Qualifications

• Proficiency in Social Work and Medical Social Work
• Experience in Case Management
• Strong communication and interpersonal skills
• Ability to work collaboratively with a multidisciplinary team
• Master's degree in Social Work and relevant licensure
• Experience in a senior care setting is a plus
