Job Summary
The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time TH Senior Pharmacist / Pediatric Clinical Pharmacist. The successful candidate will:
Dispense, compound, admix drugs, and other pharmaceuticals. Provide medication counseling and reconciliation. Review medication orders for safety and efficacy and then approves order within our CPOE system to maintain med profiles. Review and approve Pyxis profile orders. Admix oncology, TPN and investigational drug solution as needed. Dispense controlled substances as needed and maintain files. Participate in drug utilization review studies, QA and PI programs. Actively participate in new technologies and operational programs. Maintain the records of Pharmacy. Supervise technicians and support staff. Report to the Director and Associate Director of Pharmacy. Provide in-service education to healthcare professionals on demand. Rotate with medical teams and/or participate is specific medication management programs. Perform holiday coverage rotation and on-call/recall rotation as needed/required.
Required Qualifications
PharmD from an accredited college of Pharmacy. New York State Pharmacist Licensure.
Preferred Qualifications
Prior hospital pharmacist experience. PGY-1 completed residency.

Variable Days; 8:00am to 4:30pm
