Overview Children's Outpatient Services provides treatment for children and adolescents and their families and is intended to improve and maintain a child’s or an adolescent’s stability in the community. Treatment focuses on problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Services include individual, family and group therapy, medication evaluation and management, case management, in home intervention, case consultation and summer day treatment services. The type of treatment is highly variable and depends upon the individual’s existing symptoms and developmental level.

Responsibilities Primary Job Functions:

Care coordination Responsibilities:

Completes a comprehensive home-based assessment of the consumer’s needs and strengths utilizing the CANS within 14 days of intake

Identifies goals and plans for appropriate services based on the CANS

Develops Individualized Service

Plan (ISP) within 30 of first face-to-face meeting

Intensively collaborates with the treatment team members and outside sources in the delivery of care coordination

Provides appropriate linkage to community resources

Provides ongoing monitoring of consumers’ progress and needs as assessed in the CANS

Advocates on behalf of the consumers to secure community resources

Evaluates the consumer for discharge planning

Effectively demonstrates the application of child/adolescent specific competencies

Case Load Management:

Effectively manages caseload based on consumer needs

Meets AltaPointe productivity standards

Supervision and Consultation:

Seeks clinical supervision and consultation as needed

Accepts and employs suggestions for improvement

Actively works to enhance knowledge and skills

Clinical Record Keeping:

Documents in a timely fashion per AltaPointe policy

Documents appropriate, complete, and comprehensive CANS assessments

Documents measurable and specific goals in the individualized service plan

Documents in a clear and concise manner, types of consumer problems, ISP goals addressed and services provided

Documents case disposition, case closings, follow-up and referrals

Documents legibly

Courteous and respectful attitudes towards consumers, visitors and co-workers:

Treats consumers with care, dignity and compassion

Respects consumer’s privacy and confidentiality

Is pleasant and cooperative with others

Assists consumers and visitors as needed

Personal values don’t inhibit ability to relate and care for others

Administrative and Other Related Duties as Assigned:

Actively participates in Performance Improvement activities

Actively participates in AltaPointe committees as required

Completes assigned tasks in a timely manner

Follows AltaPointe policies and procedures

Attends to required in-service training and other workshops

Qualifications Bachelor’s degree preferred in Social Work (BSW). Hospital Case Management and/or discharge planning experience preferred.

Must be 23 years of age or older with a valid Alabama Driver’s License and proof of current liability insurance.

Knowledge in adult specialty, outlined below, to be attained within (6) months of employment:

Knowledge of psychiatric disorders in the adult population

Knowledge of Intellectual and Developmental Disabilities

Knowledge of Substance Use Disorders

Knowledge of Housing programs

Knowledge of VA programs

Knowledge of area shelters

Knowledge of Intellectual Disability placement services

Knowledge of inpatient substance abuse services

Basic knowledge of the nature of addiction for referral purposes

Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as to non-violent crisis intervention practices

Knowledge of community resources

Knowledge of nursing home and assisted living facility admission requirements

Knowledge of cultural diversity

Knowledge of educational methods appropriate to adults with psychiatric disorders

Knowledge of psychiatric rehabilitation principles

Knowledge of involuntary commitment laws, procedures and court policies
