Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Rehabilitation Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1 year of experience in rehabilitation or long-term care required License: Occupational Therapy license required Certifications: None required Description: Effectively screens and evaluates patients with cognitive and/or activities of daily living (ADL) physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders. Administers various procedures as part of the rehabilitation plan, including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, and modalities. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluations. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided pursuant to regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Engages in facility and company required trainings, in-services, and conferences. Able to obtain any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports occupational therapy assistants, rehabilitation aides, and students. Adheres to practice acts, codes of conduct, and ethics during professional practice.
