Job Title: Physical Therapist Profession: Physical Therapist Specialty: Rehab Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum of 1 year required; new graduates accepted with Level II experience. License: Missouri State License Required Must-Have: LTC or Rehab experience required. Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and utilization review meetings. Engages in facility and company-required training, in-services, and conferences. Obtains necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and students. Adheres to APTA and state-required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
