Job Title: Physical Therapist Profession: Physical Therapist Specialty: Rehab Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 1 year of Physical Therapy experience required License: Missouri State License required Certifications: Required Must-Have: - LTC or Rehab experience required - New graduates are welcome as long as they have Level II experience Description: This position follows a clinical interview process. Candidates can expect outreach within two hours via phone and text where applicable. Decisions are typically made the same day or by the next business day. Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan. Procedures may include manual techniques, ambulation, therapeutic exercises, modalities, and wound care. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Services are modified based on patient evaluation, and referrals are made in relation to outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Engages in facility and company-required trainings, in-services, and conferences. Obtains necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to all required practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist.
