Roles & Responsibilities – Inpatient Pharmacist Review, interpret, and verify physician medication orders for accuracy and appropriateness. Prepare, compound, and dispense medications in accordance with established protocols. Monitor patient medication therapies, identify drug interactions, and adjust dosages as needed. Collaborate with physicians, nurses, and other healthcare staff to optimize patient care. Provide drug information and education to healthcare providers and patients. Ensure compliance with hospital policies, state, and federal regulations. Manage controlled substances and maintain accurate medication records. Participate in clinical rounds, antimicrobial stewardship, and medication reconciliation. Support transitions of care through discharge counseling and medication education. Supervise and guide pharmacy technicians and interns in daily activities.
