Travel Physical Therapist (PT) - $2,486 per week in Rock Hill, SC

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 8-hour Day Shift Hours per Shift: 40 Experience: Minimum of 1 year required; previous experience in home health preferred. License: State Licensure required Certifications: BLS certification required Must-Have: 1 year of experience is required. Charting experience with Homecare Homebase is required (no exceptions). Experience with geriatric/adult chronic disease management and maintenance notes. Skills in IV, pleurx, wound vac, wound care, assessment, and disease teaching are essential. Experience with OASIS-E (assessment tool) is required. Description: Provide physical therapy services in a home health setting. Manage a typical caseload of approximately six visits per day. Work with adult patients with chronic diseases, focusing on wound care and related therapies. Utilize Homecare Homebase for charting and documentation. Coordinate care and develop treatment plans tailored to individual patient needs. Demonstrate effective disease teaching and support for patient management at home. Maintain professionalism and adherence to clinical guidelines in all patient interactions. Participate in occasional floating assignments within the designated service area. Work according to scheduled shifts, and be prepared to work holidays as needed. Wear designated scrub color and adhere to departmental policies.
