The ideal candidate will be responsible for providing home therapy services. You will manage a caseload with approximately six patient visits daily. Experience is essential for effective patient assessment and treatment. Candidates should be seasoned in home health practices. A valid driver’s license and proof of insurance are required. Support will be provided by departmental leadership. You will engage in patient education and chronic disease management. A weekend rotation is not required for this position. On-call duties are not anticipated unless specifically requested.
