As an HHA Coordinator, you will serve as key support for the Home Care Operations team, managing the HHA staffing systems and assigning staff to patient cases. Position Details
Full Time — Your Choice At Home is an affiliate of Cobble Hill Life Center. This role supports Home Care Operations and staffing processes. Responsibilities
Maintain HHAeXchange (home health aide vendor) system: broadcast HHA cases for staffing, update HHAX with patient demographics, authorization, and recertification information Collaborate with licensed home care agency (lhcsa) coordinators to ensure scheduling, billing, and authorizations are accurate Collaborate with office/field staff on changes to HHA patient care Troubleshoot lhcsa vendor issues, manage HHAX communications, and update HHAX with provider rate increases Notify lhcsa vendor of issues regarding aide services Process un-billing requests Run weekly expiring authorization reports to ensure services continue in accordance with the POC and NYS Department of Health regulations Maintain Careficient (patient care) software: assign vendors to patient schedules, collaborate with nursing supervisor and nursing staff to ensure accurate information reflected on 485, HHA POC, and care needs Run missed visit reports, remove missed visits from patient schedules, scan and upload missed visit notes Send weekly expiring authorization emails to intake staff and nursing staff Assist with staffing patient visits to subcontracted agencies (PT/OT/ST), including sending/retrieving all required documentation for patient management Qualifications
High school diploma; college degree preferred Excellent communication and customer service skills, both written and verbal Tech savvy, with proficiency in Microsoft Office, including Excel Prior experience with HHAeXchange and Careficient a plus Absolute integrity and sufficient maturity to handle confidential aspects of work What We Offer
