Primary Care Physician (PCP)
is expected to demonstrate leadership, accountability for outcomes, and a commitment to high-quality, cost-conscious care within a team-based, patient-centered model.
Responsibilities
The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults, taking full accountability for patient care and outcomes and seeking appropriate specialist consultation when needed while remaining involved in the patient’s comprehensive care. Engage with hospitalists when one of their patients is hospitalized, regardless of whether the hospitalist is part of ChenMed. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute and complex chronic health needs. Lead the care team (care promoter, care facilitator, care coordinator) for patients who visit the office; engage the transitional care team and other resources for patients unable to come to the office (in hospital, SNF, LTC or homebound). Participate in center management and cover for other providers as needed; actively participate in recruiting patients and providers as required by the market. Perform other duties as assigned by management.

PCPs work with center and market leadership and care teams toward defined metrics, including patient admissions per thousand, appointment slot utilization, patient experience, clinical gaps closures, and medical cost measures. PCPs are expected to champion a positive culture of love, accountability, and passion within the center.
Knowledge, Skills & Abilities
Availability and accessibility for patients to build trust, including openness to walk-in visits and timely communication. Service orientation: provide care as you would want for a family member or yourself. Evidence-based medicine with awareness of lifestyle and nutrition influences on health outcomes; stay updated on clinical, nutritional, and lifestyle-based interventions. Leadership: continuously develop leadership skills for patients, teams, centers, and the company. Quality focus: work with care teams to achieve high-quality outcomes and seek continuous improvement in practice and operations. Influence: competently and compassionately influence patients, teams, and oneself to achieve the best outcomes. Self-care: prioritize physician wellness to sustain patient, staff, and patient health. Proficiency in Microsoft Office Suite and relevant software; ability to travel locally and nationally for training and recruiting as required. English fluency (verbal and written); other language requirements may be specified at the time of application. Ability and willingness to exercise independent judgment.
Education And Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Active or readily obtainable State Medical License for the state(s) of employment. Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board eligibility is required. Maintain board certification post-approval, including MOC, CME, and/or retaking exams as required. Current DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.
