Primary Care Physician (PCP) Role In our organization, the PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. The PCP acts as a health coach, simplifying and prioritizing care to make behavior change actionable, and leads care teams toward their performance goals. The PCP will work closely with managers and medical directors to ensure compliance with guidelines and participation in risk and quality management programs.

Key Responsibilities:

The PCP independently provides care for patients with acute and chronic illnesses, primarily older adults.

The PCP takes full accountability for patient care and outcomes, seeking specialist consultation when needed.

The PCP engages with hospitalist and transitional care teams for patients in hospitals, SNFs, LTC, or homebound.

The PCP leads their care team consisting of medical assistant, facilitator, and coordinator for office-based patients.

The PCP manages care coordination, health education, and documentation for complex chronic health needs.

The PCP covers for other providers as needed and assists in recruiting patients and new providers for the center.

The PCP performs other duties as assigned and modified at manager’s discretion.

Availability and Accessibility – PCPs are available for walk-in visits, phone calls, and messages in a timely manner.

Evidence Based Medicine – Stay updated on clinical, nutritional, and lifestyle interventions.

Physician Leadership – Continuously develop and improve leadership skills for patients, teams, center, and company.

Quality – Work with care teams toward high quality outcomes and continuously improve practice and center operations.

Influence – Competently and compassionately influence patients, teams, and self to achieve the best outcomes.

Self-Care – Prioritize personal well‑being to care for patients effectively.

Proficient in Microsoft Office Suite products and other software used in the company.

Ability and willingness to travel locally as needed, including training and recruiting.

Fluency in English, verbal and written. Other language fluency may be required at the time of application.

This job requires use and exercise of independent judgment.

Education and Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.

Must be able to obtain a current State Medical License or already have an active State Medical License for the state(s) in which the PCP will be working.

Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred. Board eligibility required. Once board certified, PCP will maintain board certification by completing necessary MOC, CME, and/or retaking board exams as required.

Must have a current DEA number for schedule II–V controlled substances.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment.

Pay Range $214,700 - $306,714 Salary
