Licensed Practical Nurse (LPN)
to join our growing
Special Medical Team . This is an opportunity to use your clinical skills in a meaningful way; coordinating care, supporting families, and ensuring children with special medical needs in foster care receive the healthcare they deserve. What You’ll Do
Coordinate medical, dental, and behavioral health services for children in foster care. Act as the key liaison between providers, pharmacies, case managers, and families. Conduct monthly home visits to assess well-being and support caregivers. Ensure timely follow-up on appointments, referrals, and hospitalizations. Document care and treatment plans in NextGen EHR. Collaborate with HSVS’s 29-I clinic and case planning teams to set clear health goals. Qualifications
NYS Licensed Practical Nurse (LPN) (required). At least 1 year of healthcare case management or child welfare experience. Knowledge of foster care health requirements and medical terminology. EHR documentation experience (NextGen a plus). Strong communication and teamwork skills. Why Join Us?
