GENERAL SUMMARY/ OVERVIEW STATEMENT: Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists provide direct patient-centered care and integrated pharmacy operational services in a practice setting with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. They are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are integrated throughout the institution. Responsibilities include assessing patient needs, considering age and disease-specific characteristics in drug therapy and patient education, adjusting care based on patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as departmental resources and liaisons to other departments and external groups. They manage and facilitate appropriate use of medications according to professional and regulatory standards, conduct clinical research and practice advancement projects, and support quality and regulatory compliance initiatives to improve medication-use processes. They provide medication and practice-related education/training and may precept pharmacy residents and Doctor of Pharmacy students. Principal Duties and Responsibilities

Inpatient/Ambulatory Oncology Clinical Pharmacist: An Outpatient Oncology Clinical Pharmacist collaborates with and serves as a resource for nurses, physicians and other clinicians involved in the management of oncology/hematology patients and others at the MGH Cancer Center. Responsibilities include coordinating and delivering pharmacy services in compliance with applicable policies, regulatory requirements, and laws. Job-specific responsibilities include: Reviewing and monitoring patient treatment (chemotherapy and ancillary) medications for appropriateness based on current clinical and institutional guidelines and patient-specific criteria. Monitoring patients’ medication regimens for drug interactions, toxicities, and cost-effectiveness. Assessing all medication orders to ensure they meet established standards and practice guidelines; report discrepancies to appropriate staff promptly. Advising clinicians on drug selection based on current guidelines to optimize therapy. Overseeing safe handling and preparation of medications, including IV chemotherapy, IV antiemetics, IV steroids, and investigational chemotherapy drugs for outpatient and inpatient use. Checking admixtures for accuracy of preparation and labeling. Overseeing technician activities and performing quality control checks in conjunction with the Senior Attending Technician. Accessing current clinical trial documents to ensure accuracy when reviewing trial orders and maintaining investigational medication logs. Maintaining current patient information files and records in the pharmacy department to support safe and consistent drug administration. Adhering to safety requirements for handling and preparation of chemotherapy and hazardous agents according to USP 797, ASHP, NIOSH guidelines. Overseeing ordering, storage and handling of medications and supplies. Participating in education activities such as in-services and providing drug information as needed. Participating in Safety Reporting System; identifying and reporting therapy errors and adverse drug reactions. Participating in training, education and mentoring of other pharmacists and technicians, and precepting pharmacy students. Participating in area-specific staff and department meetings and contributing to quality, safety and other project work outside regular duties. May participate in research activities with publication or presentations at local and/or national meetings. Assisting in policy and procedure development and educating patients and families on chemotherapy goals, administration schedule, and toxicity management; documenting activities and interventions in the medical record. Supervisory Responsibility : Supervise pharmacy technicians in area of practice; may address personnel issues in lieu of the supervisor. Fiscal Responsibility : Enforces the formulary system and minimizes drug waste. Qualifications

BS or PharmD degree in Pharmacy. Valid license to practice as a pharmacist in MA. Completion of ASHP-accredited PGY1 Pharmacy Residency preferred. Clinical pharmacy experience (2-4 years) in a hospital setting preferred (required if no PGY1 residency). Exceptional aptitude or knowledge may be considered in lieu of experience; extensive clinical experience and development may be considered in lieu of formal training. Skills/Abilities/Competencies: knowledge of contemporary hospital/clinical practice; regulatory compliance; strong communication; ability to interact with staff; ability to integrate services with QA, teaching, research and professional development; extensive clinical knowledge in therapeutics and patient-centered care; expertise in specialty medication therapy management; strong teaching and precepting skills.
