The Nurse Navigator coordinates care across the continuum, provides education and information to patients and families, facilitates decision making, advocates for patients, and aims to improve throughput and patient satisfaction. The Nurse Navigator supports individuals and families throughout the continuum of care across all settings, monitors patient outcomes, and participates in quality improvement activities. Works in a multidisciplinary setting to positively impact patient outcomes and experiences. Estimated salary range for this position is $79,331.20 - $105,510.50 per year, depending on experience. Responsibilities
• Coordinate care across the continuum and provide education and information to patients and families. • Facilitate decision making, advocate for patients, and improve throughput and patient satisfaction. • Monitor patient outcomes and participate in quality improvement activities. • Collaborate with the health care team in a multidisciplinary setting to positively impact outcomes and experiences. Qualifications
Bachelor of Science in Nursing (BSN). Licenses & Certifications Basic Life Support (BLS). Registered Nurse (RN). Advanced Cardiovascular Life Support (ACLS). Additional Qualifications Must have an active Registered Nurse license. Three years of clinical experience in a hospital, managed care environment or medical office. Proficient in computer applications and typing skills. Experience with Electronic Medical Record systems preferred. Bilingual preferred. Preferred certification in area of expertise. For South Entities, RNs hired after 2/2012 with an Associate degree have 5 years to complete the BSN degree. Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard. Minimum Required Experience: 3 years EOE, including disability/vets
