Job Summary
The Case Manager will provide case management and supportive services for the FYI Rapid Rehousing program which secures stable housing for Foster youth between the ages of 18-24 who are eligible under the Family Unification Program.
Essential Duties/Responsibilities
Provide case management and supportive services to youth in program. Document and verify client eligibility. Conduct re-certifications for clients at least annually. Assist youth in developing and implementing an "Individualized Action Plan" to meet short and long-term needs. Maintain statistical data and complete reports as appropriate to evaluate effectiveness of services delivered. Enter data in AWARDS, HMIS and complete exit assessments as required. Ensure that client and program records are maintained and up to date, ensure data entry is timely and provide reports as required. Assist and support youth with housing searches. Develop relationships with landlords and provide mediation between landlords and families as needed. Participate in agency, program, OSH and community meetings as appropriate. Regular attendance is a requirement of the position. Other duties may be assigned.
Education And Experience
Bachelor's degree (B.A. B.S.) from four-year college or university (preferably in Counseling, Social Work or related field or equivalent combination of education and experience. Housing needs and assistance experience required. Experience working with culturally diverse young adults required.
Required Skills/Abilities
Driving is a requirement of the job. Must have possession of current driver’s license, access to a vehicle and personal automobile insurance. Current CPR and First Aid Certification required (or complete within 30 days of hire). Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations. TB test required.
Ability to work with basic mathematical concepts as required for reporting.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Internet research skills are a plus.

Salary Range
$25.00 - $27.00 per hour/year.
