Job Details
What You Will Be Doing
- Field RN seeing patients in their homes doing OASIS admissions, assessments, providing clinical care as needed, resumptions, discharges
- Provide patient care coordination working with an interdisciplinary healthcare team
- Clinical care encompasses wound care, medication administration, injections
- Provide patient teaching and education
Skills You Need
- Active and unrestricted NYS RN license
- Home Health experience preferred but not required
