This position involves travel for fieldwork, including in-home visits. Position Purpose:
Develops, assesses, and facilitates complex care management activities for members with primarily mental and behavioral health needs to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families related to mental health and substance use disorder. Evaluates member needs via phone or in-home visits, recommends and facilitates care plans/services, including behavioral health and social determinants. Performs outreach through telephonic, digital, home, or other site visits to assess needs and collaborate with resources. Develops ongoing care plans for high-acuity members, identifying providers, specialists, and community resources for mental health and substance use disorders. Coordinates between members, families/caregivers, community resources, and care providers to ensure access to services. Monitors care plans and member outcomes, adjusting as needed based on treatment side effects, complications, and symptoms. Facilitates care coordination and collaborates with providers to ensure timely access to care. Documents and maintains member information and activities to ensure compliance with regulations. Provides education to members and families on procedures, treatment options, and healthcare benefits, including behavioral health and social needs. Provides feedback to leadership on opportunities for care and quality improvement. Performs other duties as assigned and complies with policies and standards. Education/Experience:
Master's degree in Behavioral Health, Social Work, or Nursing from an accredited school, with 2-4 years of related experience. License/Certification:
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, based on state requirements, with BH experience. For Sunshine Health (FL) Only:
Master's in social work or related field and one year of pediatric care experience; may require up to 80% local travel. Pay Range: $55,100 - $99,000 per year. Centene offers benefits including competitive pay, health insurance, 401K, stock purchase plans, tuition reimbursement, paid time off, holidays, and flexible work arrangements.
