Position Summary

We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to join the Adult Medicine Clinic/Outpatient unit. The LPN will provide high-quality patient care in an acute outpatient setting, supporting physicians and RNs in delivering safe and efficient care to a diverse patient population.

Key Responsibilities

• Deliver direct patient care, including vital signs, medication administration, and patient monitoring.
• Assist with examinations, procedures, and treatments under the supervision of physicians and RNs.
• Document patient information accurately using the Allscripts charting system.
• Educate patients and families on care plans, treatments, and health maintenance.
• Maintain compliance with infection prevention, safety, and regulatory standards.

Required Qualifications

• Active LPN/LVN license (State of New York).
• Minimum 1 year of clinical experience required.
• Current BLS certification .
• Strong communication, organizational, and patient care skills.
• Ability to work effectively in a clinic/outpatient acute care setting.
