Advanced Practice Provider - Geriatrics
Location: Norwich, CT
Shift Detail: Monday-Friday
Overview:
An excellent opportunity for a Nurse Practitioner or Physician Assistant to join a growing geriatrics team dedicated to providing compassionate, value-based care to older adults in Eastern Connecticut. This position focuses on delivering care to patients in home-based and community settings, such as Assisted Living and Congregate Housing facilities.
Position Highlights:
•Provide comprehensive geriatric care through a home-based care model designed to enhance quality of life and independence for older adults.
•Collaborate with a multidisciplinary team of healthcare professionals to coordinate and manage patient care.
•Full-time, weekday schedule with flexibility in locations based on the candidate's home base.

•Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA).
•Setting: Geriatric Medicine - home-based and community settings.
•Schedule: Full-time, weekdays.
Qualifications:
•Unrestricted State of Connecticut medical license (or eligibility to obtain).
•Training as an APRN (Family, Adult, or Geriatric Nurse Practitioner) or Physician Assistant.
•Minimum of two years of post-graduate experience as an APRN or PA preferred.
•New graduates with relevant nursing or geriatrics experience are encouraged to apply.
