Overview

AdventHealth East Orlando is a 265-bed community hospital serving East Orlando since 1941. The role described is Social Work Specialist who intervenes with patients with complex psychosocial needs, assists with eligibility determination for social programs and funding sources, coordinates with community funds, and supports transitions of care or discharge planning. The position includes crisis intervention for patients and families and development of discharge plans for high-risk populations. Referrals come from interdisciplinary team members including physicians, RN care managers, staff nurses, and others.

Responsibilities

• Provide psychosocial assessment and interventions for patients with complex problems, including abuse and neglect cases as appropriate.
• Provide consult services for patients lacking decision-making capacity and coordinate with Care Management leadership per guardianship policies and procedures.
• Provide consult services for foster care and adoptions.
• Receive referrals for psychosocial issues from the health care team and deliver appropriate interventions.
• Support discharge planning and transitions of care for at-risk populations in collaboration with the care team.

Qualifications

• Bachelor\'s degree in Social Work
• Preferred: Minimum 1 year of hospital/medical social work experience
• Master\'s degree in Social Work
• Care Management discharge planning experience
• Knowledge of state and federal guidelines pertinent to care management
