Overview

Actify Home Care is a home health/home care agency covering Broward County. We are clinician-owned & operated, recognizing and rewarding hard work and celebrating team spirit with understanding. We are hiring registered nurses to provide in-home care with a focus on improving lives for our patients and for our staff.

Responsibilities

• Provide high-quality, one-on-one patient care in patients' homes.
• Collaborate with the primary doctor, Clinical Team Director, Director of Nursing, and other team members to manage and coordinate patient care.
• Advocate for patients and seek ways to maximize outcomes and safety.
• Communicate effectively to support the patient’s care plan and ensure continuity of care.

Qualifications

• RN License (Required)
• CPR/BLS Certification (Required)
• Driver's License (Required)
• Spanish speaking a plus
• Nursing experience in home health or similar settings is desirable

Work Location

On the road; serving Broward County, FL (north Broward; areas east of I-95 in Pompano Beach, Fort Lauderdale, Oakland Park, and Dania Beach).

Pay

Pay ranges from $35.00 - $55.00 per visit, with $70.00 for SOC Oasis visits; additional hourly pay possible for certain roles.
