What You’ll Do

• Own your own patient panel
• Manage chronic conditions + do full assessments
• Close care gaps (HEDIS and STAR measures)
• Lead the care planning with your team

You’ve Got To Have

• Active, unrestricted APN license
• Board certified + Medicare credentialed
• 6+ months experience in skilled nursing or long-term care
• Comfortable with digital tools + working with a team
