What You Will Do:

You will help implement personalized care plans that address the whole person—mind, body, and spirit. You will treat clients like family, going above and beyond to enhance their lives.

Essential Functions:

• Deliver exceptional care in clients’ homes
• Execute personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and emotional support
• Assist with personal care activities, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety and client enjoyment
• Provide transportation as needed
• Document tasks and report changes in client condition
