The Center for Urban Community Services (CUCS) is a nationally recognized human services organization and one of New York City's largest providers of supportive housing and social services. CUCS develops and delivers comprehensive programs that integrate housing, healthcare, and social services to support people experiencing homelessness, mental illness, substance use, and other complex challenges.
As the lead agency of the Manhattan Outreach Consortium (MOC), CUCS coordinates a citywide effort to engage and house individuals living unsheltered in Manhattan. MOC's newly expanded Care Coordination Program at Bellevue Hospital strengthens our ability to support individuals through seamless connections between street outreach and hospital-based care.
Position Summary:
CUCS is seeking a dynamic and experienced Clinical Supervisor to lead a specialized new team embedded within the MOC, providing care at Bellevue Hospital. This is a key management position responsible for overseeing day-to-day clinical and operational functions of a small but high-impact team serving individuals experiencing street homelessness. The Clinical Supervisor is the primary liaison between Bellevue Hospital and the MOC programs, responsible for triaging referrals, streamlining communication, and facilitating coordinated care transitions.
This position requires a seasoned professional with strong clinical acumen, program management experience, and a demonstrated commitment to evidence-based, person-centered, and trauma-informed care. The successful candidate will exhibit strong leadership, administrative skills, and the ability to thrive in a fast-paced and evolving service environment.
Key Responsibilities, include but are not limited to:
Provide direct supervision, mentorship, and professional development to a team consisting of one Mental Health Clinician and one Case Manager. Serve as the single point of contact between Bellevue Hospital and MOC teams to ensure timely and effective referral management, care planning, and service linkage. Oversee day-to-day operations of the Care Coordination team, including scheduling, documentation, crisis intervention, and inter-agency communication. Ensure team performance aligns with program goals and funder requirements through monitoring, coaching, and quality assurance practices. Foster strong relationships with internal and external stakeholders, particularly hospital partners, to promote collaborative care and successful client outcomes. Utilize a harm reduction, housing-first approach in supervising service delivery and engage in client-facing work as needed to support staff and address complex clinical needs. Actively participate in program leadership activities including contract compliance, data tracking, continuous quality improvement, and team culture building. Maintain flexibility to perform services in a variety of settings, including the hospital, street, shelters, supportive housing, and other community locations. Please note:
This is a one (1) Year, limited-funded position, with plans to extend if funding is available Continued employment is contingent upon ongoing funding and may end at any time based on the availability of fund resources. There is one (1) 10pm - 6am shift, per year required with this position. As needed this position will receive weekly clinical supervision that meets the requirements for LCSW hours and will have access to all CUCS agency trainings. Requirements
