Job Description:
As a Project Manager, you will lead the Care Case Management workstream for the Client. This initiative involves working closely with Client and a third-party vendor to develop a Microsoft Power Platform-based system aimed at modernizing and automating person-centered service planning for individuals with intellectual disabilities. The role requires strong leadership, communication, and technical project management skills within a healthcare or human services environment.
Key Responsibilities:
Gain understanding of the Developmental Services, its mission, and the populations served Collaborate with stakeholders including case managers, families, support providers, and end users Learn current systems, identify requirements, perform gap analysis, and manage end-user communication Lead and manage application and data migration projects within a Care Case Management environment Communicate effectively with business owners, IT program teams, and leadership Facilitate meetings, testing sessions, and test preparation activities Create and maintain project schedules, status reports, and meeting agendas Ensure HIPAA compliance and manage EHR, EMR, and patient case management systems Required Skills, Experiences, Education, and Competencies:
Technical project management experience in Care Case Management environments (e.g., social work, health & human services, hospital settings) Experience working with HIPAA-constrained data, EHR, EMR, and patient case management systems Proven success in managing application and data migration projects Strong verbal and written communication skills Excellent analytical, judgment, and decision-making abilities Proficiency in PowerPoint, Excel, and analytical tools Ability to collaborate across functions, sectors, and locations Strong time management skills and ability to meet deadlines Experience with technology-driven change management initiatives Nice to Have:
Experience using Monday.com for project management Familiarity with person-centered practices or strategies in healthcare or human services
The hourly range for roles of this nature are $50.00 to $80.00/hr. Rates are heavily dependent on skills, experience, location, and industry.
