Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 Weeks Shift: Monday to Friday, Days Hours per Shift: 32 Experience: SNF experience highly preferred License: License verification required Certifications: Negative drug screen results, LA Fire Safety Card (every 4 years) Must-Have: - Rehab Optima experience preferred - Completed credentialing checklist Description: Make sure prescreen is filled out and attached upon submitting a candidate. Resume and license verification also need to be attached. If these items are not completed and attached, the candidate will be declined. Candidates must provide a minimum of three references. A county criminal background check (7 years for all counties lived and worked) is required. A federal criminal background check (7 years or more) is required. A multi-state criminal search (7 years or more) is required. A nationwide sex offender search (7 years or more) is required. HHS/OIG and GSA searches must be conducted. A social security number trace is required. A one-step PPD or Quantiferon or proof of positive PPD and chest X-ray must be submitted. All candidates must be screened for substance use.
