Overview
The Physician Assistant (PA) at IMAN Health Center plays a critical role in delivering compassionate, high-quality, and culturally competent care to underserved communities. Working in a collaborative, team-based environment, the PA provides primary and preventive care services, supports chronic disease management, and promotes wellness in alignment with IMAN’s holistic model of care and mission of fostering health, healing, and social justice. Key Responsibilities
Clinical Care: Provide direct patient care, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions, within the scope of licensure and in collaboration with supervising physicians. Clinical Care: Conduct comprehensive health assessments, including physical exams, medical histories, and screenings. Clinical Care: Order and interpret diagnostic tests; initiate and manage treatment plans. Clinical Care: Educate patients and families on treatment plans, medication use, preventative care, and lifestyle modification. Clinical Care: Manage urgent care needs when appropriate and triage patients as necessary. Care Coordination & Team Collaboration: Collaborate with physicians, nurses, behavioral health providers, case managers, and community health workers to ensure coordinated and integrated care. Care Coordination & Team Collaboration: Participate in case reviews and multidisciplinary team huddles. Care Coordination & Team Collaboration: Refer patients to specialists or community resources as needed, following up to ensure continuity of care. Documentation & Compliance: Maintain accurate, timely, and complete electronic health records in accordance with clinical protocols and regulatory standards. Documentation & Compliance: Comply with all federal, state, and organizational guidelines, including HRSA and FQHC requirements. Documentation & Compliance: Participate in quality improvement initiatives and data reporting as part of value-based care. Community Engagement & Mission Alignment: Practice cultural humility and engage with patients from diverse racial, religious, and socioeconomic backgrounds with empathy and respect. Community Engagement & Mission Alignment: Participate in outreach, health education events, and wellness programs hosted by IMAN, as appropriate. Community Engagement & Mission Alignment: Support IMAN’s broader mission of promoting health, healing, and systemic change in underserved communities. Required Qualifications
Graduate of an accredited Physician Assistant program. Current, unrestricted state license to practice as a Physician Assistant in Illinois. Certification by the National Commission on Certification of Physician Assistants (NCCPA). DEA license and BLS certification required. Proficiency with Electronic Health Records (EHR). Preferred Qualifications
Minimum 2 years of clinical experience in primary care, community health, or urgent care. Experience with underserved populations or community health settings strongly preferred. Bilingual proficiency (e.g., Spanish, Arabic). Physical Requirements and Working Environment
IMAN Health Center is a Federally Qualified Health Center (FQHC) offering integrative care that combines medical, behavioral, and wellness services in a culturally affirming space. This position may require occasional evening or weekend shifts to support community events or urgent patient needs. Occasional lifting and moving of equipment or supplies up to 25 pounds.
