Licensed Practical Nurse (LPN) - Assisted Living / Memory Care

Full-Time, On-Site Compensation:
$54,000 - $68,000 annually (depending on experience and shift) + benefits
About the Opportunity
We're representing a respected assisted living and memory care community seeking a dedicated
Licensed Practical Nurse (LPN)
to join their clinical team. This role offers a meaningful opportunity to provide hands-on resident care while working within a supportive, mission-driven environment that values teamwork and compassion.
Key Responsibilities
Administer medications and treatments per physician orders. Monitor residents' health status and communicate changes promptly to the Wellness Director or RN. Supervise Resident Assistants (RAs) and Qualified Medication Aides (QMAs). Maintain accurate and timely documentation in resident records. Support care plan updates and compliance with state and facility standards. Assist with admissions, discharges, and communication with families.
Qualifications
Active
LPN license
in the state of Indiana. Current
CPR and First Aid
certification. Previous experience in long-term care, assisted living, or memory care preferred. Strong clinical judgment, communication, and leadership skills.
